Registration of birth and health insurance for your baby

Once your baby is born, there are several important things to take care of. These include registering the birth with the municipality and with your health insurance.

1. Registering the Baby with the Municipality

When you have a baby, you need to register the birth within 3 days. You have to register the birth in the municipality where the child was born. In the municipality of Rotterdam, you can do this either digitally or at a Municipal Office (Stadswinkel) in Rotterdam.

Registering at a Municipal Office

To register the birth at a Municipal Office, make an appointment online via www.rotterdam.nl/loket/geboorteaangifte. Or call 14 010.

What do you need to register the birth at a Municipal Office?

• A valid proof of identity or residence permit for yourself and the mother.

• A printout of the birth certificate from the hospital. You can find this in your patient portal on mijn.ikazia.nl.

• If applicable: declaration of acknowledgement.

• If applicable: declaration of name choice.

• If applicable: marriage certificate. Your child can only be added to a Dutch marriage certificate.

Registering Online

You can also register your baby with the municipality of Rotterdam online at any time of the day.

You can find additional information at www.rotterdam.nl/loket/geboorteaangifte or in the brochure of the Municipality of Rotterdam, which you will receive from the hospital.

When to register?

• If your baby is born on Monday: no later than Thursday

• If your baby is born on Tuesday: no later than Friday

• If your baby is born on Wednesday: no later than Friday

• If your baby is born on Thursday: no later than Monday

• If your baby is born on Friday: no later than Tuesday

• If your baby is born on Saturday: no later than Tuesday

• If your baby is born on Sunday: no later than Wednesday

2. Health Insurance for your Baby
You must register your baby with your health insurer yourself, within 6 weeks of the birth. Once your baby’s insurance number is known, please give it to the hospital.

While in the hospital, you can pass this on to the Admissions Department: +31 (0)10 297 55 00. Once back home, you can pass this on to the Patient Administration Department: +31 (0)10 297 50 00.

Please note! Registering with the health insurer is not the same as registering with the municipality.

If you do not register your baby with the health insurer, we will unfortunately have to send the hospital bill to your home address.
